INFORMATION REQUIRED FOR
MILITARY HALL OF FAME INDUCTION
- Family member or friend will provide 8X10 official portrait of individual being inducted. No snap shots are allowed.
- Family
member or friend will provide Proud Military Moms with Full name, rank, branch of service, alumni high school, and year graduated.
All inductees MUST be ACTIVE DUTY.
- Proud Military Moms will provide frame and plaque at a cost of $15.00.
- Proud
Military Moms will arrange for induction ceremony with the alumni high school designated official for the school, and inform
family member of time and date of induction ceremony. If interested in honoring a military graduate, please provide the following
information to: cheryl@proudmilmoms.com or michelle@proudmilmoms.com.
